EYE & Checkup

The normal eye is like a ball with three layers. Cornea is the clear glass like structure in front of the eye that is circular; it appears black because of the dark curtain called iris behind it.

The innermost lining of the ball is the retina or the film of the eye. The middle layer is called the uvea. The outermost layer is the sclera, the strong white wall of the eye.

The lens in front focuses all pictures onto the film or retina. This image is than carried from the retina to the brain by the optic nerve.

Lasik

sideimThis procedure involves first creating a thin flap of cornea with an instrument called the microkeratome. Followed by lifting of the flap and laser treatment to reshape the cornea. The flap is then repositioned in its original place. Improvement in vision is noted right from the next day.

The beam profile of Wave light laser has been wave front optimized, hence it removes lesser amount of corneal tissue when compared to other laser machines, thus making the treatment more safer for your eyes. Wavefront optimization also ensures that you do not experience any .

Glare or halos during the night while looking at bright lights. Wavelight laser allows multiple options for customized treatments, such as wave front guided, topography guided, Q factor guided treatments. Such treatment modalities are far superior when compared to those with other lasers such as zyoptix, custom vue, or custom cornea

Both PRK and LASIK procedures are absolutely painless and are performed as outpatient procedure using topical anesthetic eye drops. The entire procedure lasts for only 10 -15 minutes and you will be awake during the entire procedure. During the treatment you are required to look at a fixation light while the laser is performed and you will also hear the noise of the laser machine during the treatment. The actual laser treatment only lasts for 15 to 45 seconds. Following the treatment you can return home immediately. However you should make arrangements for a friend or relative to take you home after the procedure.

  • Am I eligible for LASIK?
  • While most patients are eligible for LASIK, not everyone is.
  • An ideal candidate for refractive surgery ?
  • Must be atleast 18 years of age with stable refraction for 1 year.
  • No associated eye diseases.
  • No connective tissue disorders.
  • Must discontinue contact lens wear 2 weeks prior to refractive surgery.

Eligibility also depends on the amount of refractive errors , the curvature and thickness of the cornea and a number of other factors that an ophthalmologist must evaluate. Proper patient selection is vital for successful refractive surgery and a satisfied patient.

Will my vision improve immediately?

There is a considerable improvement in unaided vision following the treatment. Following PRK the vision improves 3-4 days later once the surface healing in complete. Following LASIK this improvement is noted right from the next day. However a slight fluctuation of vision may occur over the next few months as the healing process continues.

What are the possible side effects?

Usually most patients are quite comfortable following LASIK. There is a moderate amount of pain following PRK on day 1, which reduces subsequently. There may be transient light sensitivity, glare & foreign body sensation, or dryness in the treated eye. Sometimes there may be a slight under correction or over correction following the procedure.

Will I need to wear glasses or contact lenses after the procedure?

Both PRK and LASIK are designed to allow you to discontinue the use of corrective lenses for routine daily work. In some patients with higher refractive errors corrective lenses of lower power may still be required following the treatment. In some individuals this residual refractive error can be corrected by a second refractive procedure

When can I resume normal activities?

You can return to work after 3 days if PRK has been performed. Following LASIK you can return to work the following day. While most activities can be resumed, it is better to avoid swimming, contact sports etc at least for a one-month duration.

Are the results of LASIK permanent?

LASIK permanently removes corneal tissue to reshape the eye in order to improve refraction. The physical results are permanent. However, you should be aware that since the eyes can still change with time, LASIK does not prevent presbyopia or affect this condition once it does occur.

Should I undergo LASIK?

Ophthalmologists all over the world have been reshaping the cornea using the excimer laser since the 1980s. LASIK has been performed internationally for approximately 10 years now. While refractive surgery does not promise perfect vision, it does hold the promise of reducing one's dependence on contacts or glasses. For many people the reduction will allow them to more freely pursue their hobbies, sports activities or career options.

 Cataract Surgery

image7Cataract or clouding of the natural crystalline lens can be corrected surgically by removing the cataract and replacing the same with an intraocular lens (IOL) implant. Ultrasound A Scan and keratotomy tests are used for accurate IOL power calculation.

The cataract is removed with phacoemulsification, a technique wherein ultrasonic energy isimage9 used to break and emulsify the cataract followed by removal. All this performed through an incision size of just 2.75mm. Our phaco machine, AMO Sovereign,with new white star technology allows cold phaco (minimal build up of heat during cataract surgery

Conductive KeraToplasty

Conductive Keratoplasty (CK) or Near Vision CK is the preferred choice for Refractive Surgery by all the eye surgeons for patients above 40 years of age. It is done to correct Presbyopia by reshaping the cornea with the help of Radio Frequency energy instead of a Laser or Scalpel.

What is Presbyopia ?

As we age, our eyes lose their ability to focus on near points. Reading, seeing the time on a watch, identifying the object near you or working on a computer becomes difficult. This usually begins to occur when we cross 40 years of age. It happens to everyone. Besides Reading Glasses, Conductive Keratoplasty is the only treatment available to correct Presbyopia.

Why CK ?

Near Vision CK procedure is specifically for people over 40 years of age, who want freedom from Reading Glasses or Contact Lenses.

When a patient has gone through a LASIK Surgery or has implanted multi-focal / accommodating Intra Ocular Lenses (IOLs) during Cataract Surgery and still requires Reading and Distance Glasses, then CK treatment can be rendered to cure the problem.

FDA Approved Equipment For CK

The Procedure is done with the help of View Point CK System (from Refractec) which uses the probe with a Micro thin (90u diameter) single use KERATOPLAST tip for delivering Radio Frequency energy directly to the corneal stroma.

RF is one of todays most advanced surgical technologies. In addition to its use in CK, RF technology is being used in prostrate cancer therapy, back surgery and even cardiovascular procedures.

Consultation

Your eyes will be examined using equipments as unique as the Wave front technology itself. To assist our doctor please do not wear soft/rigid Contact Lenses for a minimum period of two week beforehand, as they distort the cornea and might influence the outcome of the treatment.

If you have any old prescriptions for Contact Lenses or Glasses or details of any eye treatment you have altered over time, we can best determine how to treat them.

CK Procedure

STEP-I: Topical or Eye Drop anesthesia is applied to numb the eye and to ensure a painless procedure.

STEP-II: A circular treatment pattern is imprinted on the cornea using rinse away ink to guide the doctors treatment.

STEP-III: Using a probe, which is thinner than the strand of human hair, Radio waves are applied in a circular pattern to shrink a small area of collagen in the cornea.

STEP-IV: The circular treatment patterns will act like a belt tightening around the cornea by increasing its overall curvature to improve near vision.

AFTER THE PROCEDURE

Before you go home, our staff will give you personalized aftercare instructions and eye drops.

After the procedure, you will experience watering of the eyes for a certain time. This is a natural process, but please do not rub your eyes.

CK AFTER CATARACT SURGERY

CK is the most attractive alternative for low postoperative hyperopia in a Cataract patient after Multi-focal or accommodating IOLs implantation. It is less invasive and traumatic than bringing patients back for another Intraocular procedure.

CK AFTER LASIK SURGERY

It is also an alternative for over and under corrections following LASIK surgery or other excimer Laser treatment. 98% of Patients could see J5 (magazine-and newspaper-size print) in the eye that was treated by Near Vision CK. 87% of patients could see 20/20 in the distance and also read phone book-size prints (significantly smaller than a news print)

Is Conductive Keratoplasty (CK) permanent ?
Conductive Keratoplasty is for temporary reduction of  farsightedness. No matter what type of procedure is performed, our eyes inevitably change as we age. Conductive Keratoplasty SM (CK) can turn back the clock on farsightedness.

What will I feel during the Near Vision CK procedure?
The Near Vision CK procedure is considered painless. You will be aware of a support (speculum) which helps to hold and keep your eye open. The most common sensation which a patient experiences is a feeling of pressure on the eye only for a few seconds.

How soon will I be able to return to work?
With CK, the majority of patients are able to return to work and other normal activities on the day of their procedure. Although recovery is fairly quick, it is advisable to be careful with your eyes and avoid any over strain for a couple of days.

Can Conductive Keratoplasty (CK) be financed?
Because Conductive Keratoplasty is an elective surgery (Cosmetic), most health insurance plans do not cover it.

Retinal Diseases & Management

eye_im_1eRetinal is the light sensitive layer of the eye. Any disease affecting this layer results in visual disturbance. A complete evaluation of the retina is necessary during routine examination to identify any silent retinal problems. Proper evaluation of the retina is performed using indirect ophthalmoscopy and 78D lens.

 

Retinal holes or Tears

These problems increase the risk of retinal detachment and hence often require laser treatment or cryotherapy to seal them.

Other common conditions affecting the retina include Diabetic Retinopathy, occlusion of retinal blood vessels, bleeding inside the eye and age related muscular degeneration.

Fundus Photographs

The photographs are taken to document treatment. Patients can also keep a copy of this photograph for their records.

Fundus Fluorescein Angiography

The procedure is performed to identify the disease, know its extent and to plan treatment.

Green Diode (Iris Medical) or Argon Laser

This helps in treatment of various retinal diseases including retinal holes and diabetic retinopathy. Both slit-lamp delivery mode and the Laser Indirect Ophthalmoscopic Delivery System are available to ensure optimal treatment of all cases.

Surgical Intervention is necessary if the retina shifts from its normal position or if bleeding inside the eye persists fro long duration. The AMO Gemini, a newer generation Vitrectomy System, with the Iris Medical Green Diode Endolaser helps perform safe surgery to ensure optimal visual recovery.

Corneal Diseases & Management

Cornea is a clear glass like structure in front of the eye. Any disease affecting this structure results in loss of corneal clarity and thereby affecting vision. Corneal scarring can result from infections and trauma (mechanical, thermal or chemical). Abnormalities in corneal shape like keratoconus result from inherent structural abnormally and also affect vision.

eye_im_1ewrSome of these conditions can be managed with semi soft contact lens wear. However some of these patients require surgical intervention. Besides conventional full thickness corneal transplantation, newer modalities of treatment such as Deep Anterior Lamellar Keratoplasty (DALK) are performed for better visual outcome. The host endothelial layer is retained; hence there is no risk of rejection unlike conventional corneal grafts.

The innermost layer of the corneal may become dysfunctional in certain conditions and result in clouding of the cornea. This usually requires full thickness corneal transplantation surgery to restore corneal clarity. Newer surgical techniques such as Deep Lamellar Endothelial Keratoplast (DLEK) allow surgical replacement of the corneal endothelium without any surface incisions or sutures.

Corneal scarring can also affect newborn children and often require early surgical intervention to prevent visual deprivation and loss of visual development.

eye_im_1sadInfections of the cornea are not uncommon and require immediate attention. Microbiological tests are necessary to identify the causative organism and initiate appropriate medical therapy. Surgical interventions may be necessary in cases with advances infections.

Ocular surface reconstruction is necessary in cases following chemical or thermal injury, extensive ocular surface tumors or infections. Surface reconstruction is performed using amniotic membrane and limbal transplantation.

Dry Eye

Dryness of the eye is a common clinical entity. This can result from a variety of conditions; hence proper assessment is essential to manage this condition. The ocular surface clinic at Apollo Hospitals specializes in the management of this condition.

Excessive use of computers can result in ocular fatigue and other symptoms. Complete evaluation of the eye, along with modifications in working habits are required to manage this condition.

Uveitis

Uveitis is swelling of the uvea, the middle layer of the eye. It is usually an autoimmune problem though in some patients it may be due to infections such as herpes, toxoplasmosis or tuberculosis. It may be a one-time event or may occur repeatedly.

Untreated uveitis is a serious condition that may scar the retina cause glaucoma or cataract. Management requires detailed examination of the eye, appropriate investigations and accurate diagnosis. Each case requires a different management approach tailored to the specific features of that case.

HIV infection causes the immune system of the body to become weak. This can result in various blinding infections in the eye. Proper examination allows diagnosis and appropriate management, sometimes in consultation with our physicians. Absolute confidentiality is maintained at each step.

Refraction Errors & Correction

Slight alterations in vision should never be neglected. It’s very important to get yourself checked by trained optometrists and specialist and get your eyes evaluated. Uncorrected refractive errors can lead to impaired development of vision in children leading to amblyopia or lazy vision and squinting of eyes.

Refractive errors in the eye are of 3 types

Myopia or short sightednesseye_im_3

Refers to a state of the eye, herein the rays of light coming from a distant target, get ocused at a point in front of the retina. In this situation, minus powered glasses or contact lenses are necessary to focus this image onto the retina.

Hyperopia or long sightedness

In this situation, plus powered glasses or contact lenses are necessary to focus this image onto the retina.




Astigmatism or cylindrical power

Various methods to correct refractive errors

Contact Lenses perform the same function as spectacles but are placed in the cornea. They provide improved cosmesis and better quality of vision, especially for higher refractive errors.

Refractive errors can be corrected through surgery

LASIK is an exciting surgical procedure that is capable of correcting a wide range of near sightedness (myopia), farsightedness (hyperopia) and astigmatism. LASIK is the acronym for laser assisted in-situ keratomileusis. The name refers to the use of a laser to reshape the cornea without invading the neighboring cell layers. Lasik is currently the most commonly performed refractive procedure in the United States and in India. Lasik has been performed internationally for approximately 10 years.

It was first performed in clinical trails in the US in 1991. It was officially approved in 1996 in the U.S. It is important to note that the major components of the procedure have a long history. Ophthalmologists have been reshaping the cornea for over 50 years creating a protective layer of tissue for over 35 years, and using the excimer laser since the 1980's.

Photo Refractive Kertectomy (PRK)

In this procedure the laser treatment is directly performed on the surface of the cornea to imagereshape it for correction of refractive errors. This is usually performed for people with smaller refractive errors. The surface layer usually takes 3-4 days to heal and the vision gradually improves thereafter.

 

Laser in Situ Keretomileusis (LASIK)

This procedure involves first creating a thin flap of cornea with an instrument called the image1microkeratome. Followed by lifting of the flap and laser treatment to reshape the cornea. The flap is then repositioned in its original place. Improvement in vision is noted right from the next day.

The beam profile of Wave light laser has been wave front optimized, hence it removes lesser amount of corneal tissue when compared to other laser machines, thus making the treatment more safer for your eyes. Wavefront optimization also ensures that you do not experience any

glare or halos during the night while looking at bright lights. Wavelight laser allows multiple options for customized treatments, such as wave front guided, topography guided, Q factor guided treatments. Such treatment modalities are far superior when compared to those with other lasers such as zyoptix, custom vue, or custom cornea.

Both PRK and LASIK procedures are absolutely painless and are performed as outpatient procedure using topical anesthetic eye drops. The entire procedure lasts for only 10 –15 minutes and you will be awake during the entire procedure. During the treatment you are required to look at a fixation light while the laser is performed and you will also hear the noise of the laser machine during the treatment.

The actual laser treatment only lasts for 15 to 45 seconds. Following the treatment you can return home immediately. However you should make arrangements for a friend or relative to take you home after the procedure.

    Will my vision improve immediately?
    There is a considerable improvement in unaided vision following the treatment. Following PRK the vision improves 3-4 days later once the surface healing in complete. Following LASIK this improvement is noted right from the next day. However a slight fluctuation of vision may occur over the next few months as the healing process continues.

    What are the possible side effects?
    Usually most patients are quite comfortable following LASIK. There is a moderate amount of pain following PRK on day 1, which reduces subsequently. There may be transient light sensitivity, glare & foreign body sensation, or dryness in the treated eye. Sometimes there may be a slight under correction or over correction following the procedure.

    Will I need to wear glasses or contact lenses after the procedure?
    Both PRK and LASIK are designed to allow you to discontinue the use of corrective lenses for routine daily work. In some patients with higher refractive errors corrective lenses of lower power may still be required following the treatment. In some individuals this residual refractive error can be corrected by a second refractive procedure

    When can I resume normal activities?
    You can return to work after 3 days if PRK has been performed. Following LASIK you can return to work the following day. While most activities can be resumed, it is better to avoid swimming, contact sports etc at least for a one-month duration.

    Are the results of LASIK permanent?
    LASIK permanently removes corneal tissue to reshape the eye in order to improve refraction. The physical results are permanent. However, you should be aware that since the eyes can still change with time, LASIK does not prevent presbyopia or affect this condition once it does occur.

    Should I undergo LASIK?
    Ophthalmologists all over the world have been reshaping the cornea using the excimer laser since the 1980s. LASIK has been performed internationally for approximately 10 years now. While refractive surgery does not promise perfect vision, it does hold the promise of reducing one's dependence on contacts or glasses. For many people the reduction will allow them to more freely pursue their hobbies, sports activities or career options.

    Other Options of Refractive Surgery
    Phakic IOL implantation

    image2Higher degrees of refractive errors often cannot be corrected by laser refractive surgery, in these patients, implantation of special implants such as phakic intraocualr lens (Veriseye) allows correction of refractive errors with good unaided vision post surgery

Astigmatic Keratotomy

image3Using a diamond knife, incisions can be placed on the cornea to correct astigmatic refractive errors (cylindrical power).

Refractive lens exchange

eye2Very high refractive errors can be corrected by replacing the natural crystalline lens with new generation intraoculer lens (Wavefront optimized anterior surface multifocal lens and posteriors square edge IOL’s) i.e. refractive cataract surgery).

Cataract Surgery

Cataract or clouding of the natural crystalline lens can be corrected surgically by removing the cataract and replacing the same with an intraocular lens (IOL) implant. Ultrasound A Scan and keratotomy tests are used for accurate IOL power calculation.

The cataract is removed with phacoemulsification, a technique wherein ultrasonic energy is used to break and emulsify the cataract followed by removal. All this performed through an incision size of just 2.75mm. Our phaco machine, AMO Sovereign,with new white star technology allows cold phaco (minimal build up of heat during cataract surgery)

that is much safer for the eye compared to conventional phaco machines. IOL selection is customized for each individual to ensure best visual outcome. The surgery is painless and patients can return home the same day.
